Your diabetic patient has requested to be fit with Diabetic Footwear and would like entitlement through “The Medicare Therapeutic Shoe Bill.” This protective footwear can help in the fight to prevent the 90,000 diabetic amputations that occur each year.
“Medicare’s Therapeutic Shoe Bill” was passed in 1993 to help reduce the risk of non-traumatic foot amputations and keep the diabetic patient’s feet healthy.
Please review and sign the attached form to verify that this patient is a diabetic who would benefit from this protective footwear.
